Community Paramedicine

Community Paramedicine is a segment of Mobile Integrated Healthcare (MIH) that is a provider-led, patient-centered delivery care model using appropriately trained Emergency Medical Services (EMS) clinicians in an expanded role to render care, facilitate a more efficient delivery of care, and enhance access to community resources that address the social determinants of health.

Advanced-practice Community Paramedics extend the reach of a Member's existing physician, primary care, or direct primary care (DPC) relationship — closing the gap between office visits, after a hospital stay, or when a same-day concern doesn't need an Emergency Department.

Services list: 

• Post-discharge and transitional care follow-up to reduce hospital readmissions; 
• Chronic disease monitoring and management (diabetes, COPD, CHF, hypertension); 
• In-home vitals checks, medication reconciliation, and fall-risk/home-safety assessments; 
• Same/next-day, in-home evaluation for minor illness or injury that would otherwise mean an urgent care or emergency room visit; 
• Coordination and reach-back with primary care physicians, DPC practices, or specialists.

Access to a Community Paramedic is established through an annual health and wellness exam — not a membership or subscription fee. Because eligibility is tied to a covered medical exam rather than a retainer, Community Paramedicine is FSA/HSA eligible.

Community Paramedicine is best suited for Members managing a chronic condition, recovering from a hospital stay or procedure, aging in place, or for all persons and patients who simply want a trusted, advanced-practice clinician who can see them at home — delivered to the same Physician-directed standard as every EMERGILITY® daP subcomponent.
