Physician extension brings physician-directed care to the patient's side through EMERGILITY® EMS providers and practitioners, under the direction of our Operational Medical Director.
Physician Extension at EMERGILITY®
Advanced Practice EMS Providers, Paramedics & Practitioners
Physician extension occurs when a physician authorizes a non-physician to deliver medical care on the physician's behalf. In traditional practices, the physician extender is a nurse practitioner (NP) or physician assistant (PA) working under a collaborative practice agreement.
In EMS, physician extension happens through a licensed EMS agency. Certified EMS providers deliver care under the agency's Operational Medical Director (OMD), standing orders, and protocols. In Virginia, as in most states, a physician's license alone cannot authorize EMS care outside a licensed agency; the agency is what makes it lawful.
EMERGILITY® provides discreet, protective, urgent, and emergency medical services utilizing physicians, physician extenders (advanced practitioners, nurses, and EMS providers), mixed medical-security professionals, and non-medical security personnel. The level and type of provider present is based on client need and resource availability.

Emergency Medical Services (EMS) Provider Levels
EMS Provider Levels
In Virginia, an EMS provider is any person holding a valid EMS certificate issued by the Commissioner (§ 32.1-111.1).
Emergency Medical Responder (EMR)
Provides immediate lifesaving care with minimal equipment while awaiting additional EMS resources, and assists higher-level personnel on scene.
Emergency Medical Technician (EMT)
Provides basic emergency care and transport, stabilizing patients from routine transports to life-threatening emergencies, under medical oversight.
Advanced EMT (AEMT)
Provides basic and limited advanced emergency care and transport, using basic and advanced equipment, as a link from the scene to the hospital.
Paramedic (NRP)
An allied health professional providing advanced emergency care for critical and emergent patients, with the complex knowledge and skills it requires.
Paramedic Practitioner
Advanced practice paramedics, EMS-PAs, and EMS-NPs who extend physician-level care to the patient's side, as physician extenders in the field.
EMS-Physician
A board-certified, licensed physician providing medical oversight. In Virginia, an OEMS-endorsed physician may serve as an agency's medical director.
Operational Medical Director
"Operational medical director" or "OMD" means an EMS physician, currently licensed to practice medicine or osteopathic medicine in the Commonwealth, who is formally recognized and responsible for providing medical direction, oversight, and quality improvement to an EMS agency.
